Like some other people had already said, this makes me feel uncomfortable, which is a good thing. Beyond that I like the simple style; You can see that a lot of work was put into it, but you stopped before it got tainted by unnecessary detail.

This is intensely uncomfortable.

I don't know how much more I want to say beyond that, because that is what overpowers whatever experience I might have looking at it. I like the style a lot, and it works very well in this piece, fitting in with both the overall tone and the old photo effect. The values are nicely done on the face. The chest looks a quite washed out, but based on the subject matter, I assume the overtly high contrast was intentional? It all reminds me a bit of CassetteRecorder's work, but this is much more minimalistic and subtly executed than most of his work, and much more powerful as a result.

It makes me feel scared, uncomfortable, and a little sick, which I think means that your image succeeded fantastically.
